The admiting paperwork

Here are the documents that you will have to fill out before your first visit at the Maison Chiropratique Petits et Grands. Choose the form for the Little Ones if your child is under 12 years old, otherwise, use the form for the Tall Ones. This questionnaire is very thorough. We thank you in advance for taking the time to fill it out. All this information is crucial in order to establish what is going on with you and how we will be able to help you. Most importantly, do not forget to write the date as well as the name of the person who referred you to us so that we can thank that person appropriately. Also, do not write in the white rectangles please.
